(1)投标人所投产品若为医疗器械产品，提供所投产品医疗器械备案凭证，生产备案凭证（限一类）；产品注册证、生产许可证（限二、三类）；投标人如为代理商还需提供对应产品的医疗器械经营备案凭证（二类）、医疗器械经营许可证（三类），相关许可证件截止在开标前均在有效期内（提供扫描件并加盖公章）
